FDA Breakthrough Therapy Designation Timeline
- October 23, 2018 — COMPASS Pathways receives FDA Breakthrough Therapy Designation for COMP360 (psilocybin) for treatment-resistant depression. First BTD ever granted for a classic psychedelic. [11]
- November 22, 2019 — Usona Institute receives FDA Breakthrough Therapy Designation for psilocybin for major depressive disorder. [12]
- June 2025 — COMPASS COMP005 Phase 3 trial meets primary endpoints. First positive Phase 3 trial for any classic psychedelic. [6]
- February 2026 — COMPASS COMP006 Phase 3 trial meets primary endpoints (second consecutive). Rolling NDA submission begins. [7]
- April 18, 2026 — Executive Order 14401 directs FDA to prioritize psychedelic drug review. [13]
- April 24, 2026 — FDA issues Commissioner's National Priority Vouchers to COMPASS, Usona, and Transcend Therapeutics. [14]

Antidepressant Failure Rates (STAR*D Trial)
The STAR*D trial (Rush et al., 2006) is the largest antidepressant effectiveness study ever conducted (N=4,041). Key findings:
- Step 1 (first antidepressant): 37% cumulative remission
- Step 2 (switch or augment): ~53% cumulative remission
- Step 3 (third medication): ~57% cumulative remission
- Step 4 (fourth medication): ~67% cumulative remission
- After all steps: ~33% of patients never achieved remission
- 2.8 million Americans have treatment-resistant depression (30.9% of medication-treated MDD patients)

PSSD — Post-SSRI Sexual Dysfunction Research Summary
Post-SSRI Sexual Dysfunction (PSSD) is a condition in which sexual, cognitive, and emotional side effects from SSRI antidepressants persist after the medication is discontinued.
- EMA formally recognized PSSD in 2019 and required SSRI label updates [23]
- Estimated prevalence: 1–10% of SSRI users; highly underreported
- No FDA-approved treatments exist as of 2026
- No completed randomized controlled trials of psilocybin specifically for PSSD as of 2026
- Proposed mechanism: 5-HT2A agonism may help reset serotonin receptor sensitivity desensitized by chronic SSRI use
